A 23-year-old man started with chest pain 8 h after his first Pfizer-BioNTech COVID-19 vaccination. ECG evaluation showed sinus tachycardia with ST-segment elevation in D1, AVL, V5, and V6, the findings compatible with acute subepicardial myocardial damage. However, cardiac MRI documented myocardial fibrosis, with cardiac late enhancement non-ischemic pattern with diffuse edema. He had no other symptoms to suggest another etiology than the vaccination. The patient was hospitalized and received corticosteroid (prednisolone) daily. Then, 2 weeks after hospitalization, all laboratory parameters and ECG were normal and the patient was discharged from the hospital. The patient had a history of Wolf-Parkinson White that was corrected with ablation when he was 11 years old. This report calls attention to myocardial adverse reaction risk for mRNA COVID-19 vaccines for people with a previous cardiac disease history.

Background The aim of this study was to identify in asymptomatic patients with repaired tetralogy of Fallot the prevalence and determinants of impaired left-sided cardiac function and adverse ventricular remodeling and the relation of left ventricular (LV) dysfunction and remodeling with cardiopulmonary exercise capacity. Methods and Results In a cross-sectional study, 103 patients with tetralogy of Fallot (median age, 16.3 years) in New York Heart Association class 1, with surgical repair at a median age of 1.1 years, and 63 age-matched controls were studied. LV, right ventricular function and geometry, LV myocardial extracellular volume (n=57), and left atrial function were quantified with cardiac magnetic resonance. Peak oxygen consumption was measured by a standardized cardiopulmonary exercise test (n=70). Patients with tetralogy of Fallot had lower LV ejection fraction (P=0.001; 49% below age-adjusted fifth percentile for controls), lower LV mass index (P=0.003), lower LV mass/volume ratio (P<0.01), and impaired left atrial function. Right ventricular mass/volume ratio was the best predictor for LV systolic dysfunction and for a lower LV mass/volume ratio. Compared with controls, LV extracellular volume was higher (P<0.001), particularly in female patients, and associated with subnormal peak oxygen consumption (P=0.037). A peak oxygen consumption below the third percentile reference level was more likely with decreasing LV ejection fraction (P=0.008), and lower LV mass index (P=0.024), but independent of right ventricular ejection fraction. Conclusions In New York Heart Association class 1 patients with tetralogy of Fallot, frequent impaired systolic and diastolic LV function, LV adverse remodeling with LV atrophy, a decreased mass/volume ratio, and extracellular matrix expansion suggest cardiomyopathic changes. The best predictor for LV systolic dysfunction was the right ventricular mass/volume ratio. The subnormal peak oxygen consumption indicates that monitoring of LV status may be important for long-term prognosis.

Amazon forests, which store ∼ 50% of tropical forest carbon and play a vital role in global water, energy, and carbon cycling, are predicted to experience both longer and more intense dry seasons by the end of the 21st century. However, the climate sensitivity of this ecosystem remains uncertain: several studies have predicted large-scale die-back of the Amazon, whereas several more recent studies predict that the biome will remain largely intact. Combining remote-sensing and ground-based observations with a size- and age-structured terrestrial ecosystem model, we explore the sensitivity and ecological resilience of these forests to changes in climate. We demonstrate that water stress operating at the scale of individual plants, combined with spatial variation in soil texture, explains observed patterns of variation in ecosystem biomass, composition, and dynamics across the region, and strongly influences the ecosystem's resilience to changes in dry season length. Specifically, our analysis suggests that in contrast to existing predictions of either stability or catastrophic biomass loss, the Amazon forest's response to a drying regional climate is likely to be an immediate, graded, heterogeneous transition from high-biomass moist forests to transitional dry forests and woody savannah-like states. Fire, logging, and other anthropogenic disturbances may, however, exacerbate these climate change-induced ecosystem transitions.

BACKGROUND: Patients with hypoplastic left heart syndrome after a Norwood operation show dilatation and reduced distensibility of the reconstructed proximal aorta. Cardiac magnetic resonance imaging (CMR) and angiographic examinations indicate that the native descending aorta (DAo) is also dilated, but this has not been studied in detail. METHODS AND RESULTS: Seventy-nine children with hypoplastic left heart syndrome in Fontan circulation (aged 6.3±3.2 years) and 18 control participants (aged 6.8±2.4 years) underwent 3.0-tesla CMR. Gradient-echo cine and phase-contrast imaging was applied to measure cross-sectional areas (CSAs), distensibility, pulse wave velocity, and the incremental elastic modulus of the thoracic aorta. CSA of the DAo in patients was also compared with published percentiles for aortic CSA. Patients had significantly larger CSA of the DAo at the level of pulmonary artery bifurcation (229.1±97.2 versus 175.7±24.3 mm/m(2), P=0.04) and the diaphragm (196.2±66.0 versus 142.6±16.7 mm/m(2), P<0.01). In 41 patients (52%), CSA of the DAo was >95th percentile level for control participants, and the incremental elastic modulus of the aortic arch and the DAo was higher than in patients with normal CSAs (arch: 90.1±64.3 versus 45.6±38.9 m/s; DAo: 86.3±53.7 versus 47.1±47.6 m/s; P<0.01). Incremental elastic modulus of the aortic arch and the DAo correlated with the CSA of the DAo (arch: r=0.5; DAo: r=0.49; P<0.01). CONCLUSIONS: Children with hypoplastic left heart syndrome frequently show dilatation of their DAo associated with increased stiffness of the aortic arch. Higher aortic impedance increases the afterload of the systemic circulation and likely contributes to the burden of the systemic right ventricle.

BACKGROUND: The elastic function of the aorta in patients with transposition of the great arteries after arterial switch operation (ASO) is suspected to be important for long-term prognosis. METHODS AND RESULTS: Fifty-one patients and 34 controls were studied at 3.0 Tesla with MRI. Forty-three patients (12.8±6.9 years) underwent 1-stage ASO, 8 patients (23.8±6.9 years) had prior pulmonary artery banding (2-stage ASO). Aortic dimensions, distensibility, pulse wave velocity, aortic arch angle, left ventricular (LV) mass, LV systolic function and left atrial (LA) volumes, and LA passive emptying function as marker of LV diastolic function were assessed. Compared with controls, patients had increased aortic root areas (602.6±240.5 versus 356.8±113.4 mm(2)/m; P<0.01) and reduced distensibility of the thoracic aorta most pronounced at the aortic root (3.2±2.0 versus 9.1±4.7×10(-3) mm Hg(-1); P<0.01). Aortic distensibility correlated negatively with the aortic areas (P<0.01). Pulse wave velocity was higher in adults after ASO (5.0±1.0 versus 3.8±1.3 m/s; P<0.01). In contrast to controls pulse wave velocity and distensibility correlated with age in patients (P=0.04 to <0.01), LV mass was higher in patients (P=0.02). LA volumes correlated negatively with aortic root and ascending aortic distensibility and positively with pulse wave velocity (P<0.05). In patients, LA passive emptying function was lower (27.3±8.9 versus 41.1±6.0; P<0.01) and correlated with aortic root distensibility (P=0.004). CONCLUSIONS: Reduced aortic bioelasticity and aortic root dilatation are present in transposition of the great artery patients post ASO and are likely to contribute to LV diastolic dysfunction. Impaired aortic bioelasticity was strongly associated with age, suggesting the usefulness of follow-up studies for early onset of degenerative cardiovascular disease.

RESUMO

Objetivo: revisar a literatura acerca da estratégia de endomarketing, conhecer sua importância no processo de gestão e estimular reflexões e desafios para novas possibilidades da gestão em saúde. Metodologia: levantamento bibliográfico de publicações na base de dados Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS) e levantamento manual e eletrônico de outros periódicos não indexados. Os critérios de inclusão foram artigos científicos sobre o tema, publicados entre 1990 e 2010 em periódicos nacionais. Resultados: após análise das publicações indexadas na referida base de dados, foram encontrados 13 artigos, sendo que 5 foram considerados pertinentes à temática estudada. Observou-se uma escassez de textos relacionados à utilização do endomarketing na gestão de pessoas. Verificou-se, ainda, que a estratégia do endomarketing é uma proposta que vem ocupando o seu espaço na área da saúde, mas de forma incipiente. Conclusão: a compreensão da importância dessa nova ferramenta aplicada à saúde representa uma contribuição para um novo modelo de gestão.

BACKGROUND: Alcoholic beverages may have protective cardiovascular effects but are known to increase the plasma levels of triglycerides (TG). Both TG and the ratio of TG to high-density lipoprotein cholesterol (TG/HDL-cholesterol) are associated with increased cardiovascular risk. OBJECTIVES: To determine the predictive factors for variations in plasma levels of TG and the TG/HDL-cholesterol ratio in patients after they had consumed red wine for 14 days. METHODS: Forty-two subjects (64% men, 46 ± 9 years, baseline body mass index [BMI] 25.13 ± 2.76 kg/m(2)) were given red wine (12% or 12.2% alc/vol, 250 mL/day with meals). Plasma concentration of lipids and glucose were measured before and after red wine consumption. Blood was collected after 12 hours of fast and alcohol abstention. RESULTS: Red wine increased plasma levels of TG from 105 ± 42 mg/dL to 120 ± 56 mg/dL (P = .001) and the TG/HDL-cholesterol ratio from 2.16 ± 1.10 to 2.50 ± 1.66 (P = .014). In a multivariate linear regression model that included age, baseline BMI, blood pressure, lipids, and glucose, only BMI was independently predictive of the variation in plasma TG after red wine (beta coefficient 0.592, P < .001). BMI also predicted the variation in TG/HDL-cholesterol ratio (beta coefficient 0.505, P = .001, adjusted model). When individuals were divided into three categories, according to their BMI, the average percentage variation in TG after red wine was -4%, 17%, and 33% in the lower (19.60-24.45 kg/m(2)), intermediate, and greater (26.30-30.44 kg/m(2)) tertiles, respectively (P = .001). CONCLUSIONS: Individuals with higher BMI, although nonobese, might be at greater risk for elevation in plasma TG levels and the TG/HDL-cholesterol ratio after short-term red wine consumption.

Antiretroviral therapy (ART) in HIV-infected patients has been associated with an increased risk of cardiovascular disease. This study evaluates vascular endothelial dysfunction of the peripheral circulation in Brazilian HIV-infected subjects on ART or naive to ART compared to a control group matched for age and body mass index (BMI). We performed a cross-sectional comparative study to measure postischemic peak flow-mediated dilation (FMD) of the brachial artery and the response to glyceryl trinitrate (GTN) in HIV-infected patients and healthy controls in Salvador, Bahia, Brazil. Endothelial vasomotor function was evaluated by assessing brachial artery FMD. Forty-four HIV-infected individuals (33 ARV treated and 11 ART naive) were compared to 25 healthy controls matched for age and BMI. FMD % was significantly lower for the ART-experienced patients compared to the ART-naive patients and was also significantly different from controls (ART experienced 8.2 +/- 6.0% vs. 19.3 +/- 4.8% vs. 23.3 +/- 6.1%), respectively (p < 0.0001). The cholesterol, triglyceride, and ALT levels were significantly higher in the ART-experienced group compared to the ART-naive and control subjects (p < 0.028); however, linear regression analysis revealed a statistically significant association of endothelial dysfunction as a dependent variable only with ARV treatment in HIV-infected subjects (p = 0.03). The association of endothelial dysfunction with ARV therapy in HIV-infected patients was independent of protease inhibitor-containing regimens or dyslipidemia. This dysfunction may contribute to the risk for HIV-associated atherosclerosis.

The HIV epidemic has dramatically changed the paradigm for the development of drug therapy in the last 15 years. The goal is now not only to provide an effective reduction of plasma viremia , but also to reconstitute the immune deficiency due to the progression of the disease. Significant problems with the metabolism of sugars and lipids lead to the appearance of well-documented disorders such as insulin resistance, abnormalities in lipid metabolism and lipodystrophy in those patients on prolonged therapy with antiretrovirals. The question of whether or not HAART-associated lipid disorders contribute to the premature development of coronary artery disease is of major importance for the HIV community. Endothelial injury is associated with disease-related biochemical abnormalities that are implicated in HIV pathogenesis. The exploration of endothelial function began in the early 1980s at the start of the epidemic. The study of endothelial function in HIV infection and its modifications by HAART is an exciting new field in clinical research; in this review the available information on cardiovascular diseases associated with HIV infection and its treatment are discussed.

INTRODUÇÃO: O consumo moderado de vinho tinto está inversamente associado ao desenvolvimento de doença cardiovascular. Efeitos do vinho tinto no sistema nervoso simpático e na reatividade vascular não estão totalmente esclarecidos. MÉTODOS: Foi avaliada a atividade simpática do nervo muscular e a dilatação mediada pelo fluxo na artéria braquial por ultrassom em 10 indivíduos hipercolesterolêmicos, 9 hipertensos e 7 controles antes e após o consumo de vinho tinto durante 15 dias. Medidas hemodinâmicas foram realizadas com Finometer: pressão arterial sistólica, diastólica, frequência cardíaca, débito cardíaco e resistência vascular sistêmica foram calculados continuamente durante a microneurografia. Para avaliação da atividade simpática, utilizou-se punção de nervo fibular - esta foi medida como bursts/min durante período basal, teste do gelo e exercício estático com 30% da contração voluntária máxima. RESULTADOS: Após 15 dias de vinho tinto, a atividade simpática aumentou de forma significante em hipertensos e hipercolesterolêmicos (p < 0,05); porém, a dilatação mediada pelo fluxo aumentou somente em hipercolesterolêmicos (p < 0,05). As pressões arteriais sistólica e diastólica não apresentaram mudança significante. Não houve mudança na freqüência cardíaca. Houve aumento do débito cardíaco em controles, diminuição da resistência arterial sistêmica durante o gelo em controles no período basal. CONCLUSÃO: O consumo de vinho tinto aumentou a atividade simpática em hipertensos e hipercolesterolêmicos; porém, somente estes experimentaram melhoria da função endotelial, o que sugere diferentes mecanismos na regulação da função endotelial.


Introduction: Moderate red wine intake is inversely associated with development of cardiovascular disease. Red wine effects in sympathetic activity and vascular reactivity are not fully understood. Methods: Muscle sympathetic nerve activity and flow mediated dilatation of brachial artery by ultrasound were evaluated in 10 hypercholesterolemic, 9 hypertensive, and 7 controls subjects before and after red wine intake during 15 days. Hemodynamic measures were done with Finometer: arterial blood pressure, heart rate, cardiac output, and systemic vascular resistance were assessed during mycroneurography. The sympathetic activity was evaluated during baseline, cold test and isometric exercise. Results: After 15 days of red wine intake, sympathetic activity increased significantly in hypertensives and hypercholesterolemics (p < 0.05). On the other hand, flow mediated dilation increased after red wine only in hypercholesterolemics (p < 0.05). Systolic and diastolic blood pressure as well as heart rate did not change significantly. Cardiac output increased in controls and systemic vascular resistance decreased during cold test in controls. Conclusion: There were similar increases in sympathetic activity in hypertensive and hypercholesterolemic subjects; however, endothelial function was restored only in the latter group, which suggests different mechanisms regulating endothelial function.

O presente estudo trata de uma investigação do tipo estudo de caso, de abordagem qualitativa, com o propósito de analisar as percepções da equipe de enfermagem sobre o atendimento ao cliente em situação de parada cardiorrespiratória (PCR) em um pronto-socorro de um Hospital público de Natal-RN. A PCR é a mais grave e importante urgência clínica presente nos serviços de atendimento de urgência ou no ambiente extra-hospitalar, durante a qual profissionais de saúde ou pessoas treinadas em ressuscitação cardiorrespiratória (RCR) realizam manobras específicas de suporte básico de vida (SBV), num primeiro momento; acrescentando-se o suporte avançado de vida (SAV) que, além das manobras específicas do SBV, introduz terapêuticas e equipamentos mais complexos. Foram aplicadas 18 entrevistas semi-estruturadas, com 9 enfermeiras e 9 auxiliares de enfermagem, do pronto-socorro de um hospital público de Natal, RN, referência para todo o Estado, entre os meses de maio a agosto de 2003. Os dados qualitativos, resultantes das entrevistas, foram investigados de acordo com a análise de conteúdo temática e com o apoio do referencial teórico, que norteou o estudo, surgindo as seguintes categorias: Conhecendo a parada cardiorrespiratória, atuando na parada cardiorrespiratória e vivenciando uma situação estressante. A análise permitiu concluir que as profissionais de enfermagem sabem identificar uma situação de PCR e demonstram ter iniciativa; no entanto, a maior parte não possui o conhecimento de todas as condutas necessárias para a ocasião. Durante o atendimento ao cliente com PCR, não desenvolvem assistência sistematizada; além disso, neste momento, encontram-se sob intenso estresse devido às precárias condições de trabalho, o que causa um atendimento demorado, improvisado e com exposição excessiva dos clientes....

RESUMO

Infectious complications following heart transplantation are an important cause of morbidity and mortality. Generally, bacterial infections are predominant; however, fungal infections can be responsible for up to 25 percent of infectious events. We report the case of a patient who presented with histoplasmosis as an infectious complication five years after heart transplantation due to a chagasic cardiopathy. This association has rarely been reported in the international literature

In 1992, a dog naturally infected with Leishmania was found in a periurban area of Sabará, state of Minas Gerais, where human cutaneous leishmaniasis had been previously described. The parasite was classified as Leishmania, subgenus Viannia, which L. braziliensis, the main species of parasite present in the southeast Brazil, also belongs. In order to assess the importance of the dog in the transmission cycle of the disease, a canine survey was undertaken. Six hundred thirty-one dogs were examined and the prevalence of seropositive dogs for crude Leishmania amazonensis antigen was 3.2 per cent. The presence of infected people and seropositive dogs either near or in the same house was observed. This fact suggests some transmission in the domiciliary environment, with the dogs being a risk factor for human infection in that periurban area. In the other hand, the low percentage of seropositive dogs points towards a secondary importance of these animals in the transmission of Leishmaniasis in that recent focus of the disease.
